Acknowledge of Screening Limitations
Please be aware that this diabetic eye screening does not constitute a full eye exam, nor will any treatment be administered during the screening. The screening visit will be comprised of limited assessments including a diabetic health history, diagnostic imaging, image study interpretation, and follow up recommendations with a retina specialist. An annual comprehensive eye exam should be part of your annual healthcare routine and should be scheduled regardless of participation in this event.
